Figure 6-8.-Operator’s Daily PM Report, NAVFAC 11260/4.If involved in a mishap, your first responsibility isto render aid to the injured. After they have been caredfor, complete the mishap report. As an aid in completingStandard Form 91, comply with the followinginstructions:1. Obtain and properly spell names and streetaddresses of persons involved in the mishap and anypersonnel that may have witnessed the mishap.2. Carefully note weather conditions, roadconditions, position of the vehicle involved, and otherdetails, which you will not be able to get later.3. Be sure that your report gives a clear picture ofwhat actually happened. Your diagram of the mishapshould show exactly where the vehicles were before andafter the mishap.4. State damage you can see, such as “crushed rightrear wheel or crumpled fender,” and give an estimate ofthe amount of damage. If someone claims that he or shehas damaged property but you cannot see the damage,note on the accident form only that he or she “claimsbent fender,” and so forth. Follow the same procedureswith injuries. Report cuts, burns, broken bones, and soforth, of which you are certain, and note only that aperson “claims” an injury when you have no way ofknowing the truth.If you cannot get the exactinformation on some item, write “unknown” to showthat you did not overlook it.5. When sufficient space is not available forproviding information regarding an item, write “seeattached sheet,” and attach an extra sheet containing theadditional information on that item.6. After you have finished your report, look it overcarefully and ensure it is complete and accurate. If youare satisfied, sign the report, and take it to the mishapinvestigator.6-11
